State Department is being dismantled to

some degree it does seem that there are
00:37:36
a number of slots under Secretary Ridge
which have not been filled there are
00:37:40
a number of spots which of the assistant
secretaries the people who are the
00:37:43
so-called regional bureaus these are the
people who really make policy views of the
00:37:47
close advisors to the secretary and normal
State Department and what's puzzling to
00:37:52
people like me who look you know whether.
Or not there's been Democrats or
00:37:55
Republicans these people are the people
trained in diplomacy especially the career
00:38:00
people but also the pull of political
people who come in and they're trained in
00:38:03
a mechanism which is part of the worldwide
mechanism which allows countries to deal
00:38:08
with one another on things from climate
change to nonproliferation to bilateral
00:38:12
issues to political alliances and so the
feeling that one gets and I'm as puzzled
00:38:18
by this as anyone else is that the leadership
of the government that it's the both
00:38:22
the White House and perhaps even Mr Tillerson
seem to think that changing the way
00:38:28
not changing the policy but changing the
way policy is made is something that's
00:38:31
very necessary and that they would
appoint people until those changes the
00:38:35
structural changes are made the difficulty
here is that there seems to be an
00:38:40
assumption that what diplomats do what
they're trained to do and how they do it is
00:38:45
somehow not as important as having different
ideas and again the analogy that I
00:38:51
struggle to find is you know would you
want someone who is who doesn't have
00:38:55
a driver's license to turn or I'm trying
to say that the people who might come in
00:38:58
would be inept It's just that
their various skills that
00:39:02
a diplomat has Do you think sort of
00:39:04
a control thing that this administration
just doesn't not want anyone from any past
00:39:08
administration this whole
loyalty thing we've heard
00:39:11
a lot from this White House do you think
that that could have something to do with
00:39:15
it and does loyalty even play
00:39:17
a role in this type of situation this to
me is beyond loyalty every group that has
00:39:22
come in famously Republicans turning over
to Democrats rather Democrats turning
00:39:26
over to Republicans initially they look
at the State Department and say These
00:39:30
people worked for my predecessor I just
ran against my predecessor and then they
00:39:34
quickly realize that when
you're trained to be
00:39:36
a diplomat you're serving the country and
you've oily and very carefully and very
00:39:40
honestly serve the person who is your boss
that's your constitutional job and very
00:39:45
quickly everyone I've worked with and I
worked in the Clinton White House and then
00:39:49
went on to work in the Bush White House
they quickly realize that they have very
00:39:53
valuable piece. We're going to give them
honest straightforward vice there's
00:39:57
another element to this which is not just
00:39:59
a domestic element the way in which our
country talks to 3rd countries let's say
00:40:03
there's an issue about missiles in North
Korea or the war in Syria who talks to the
00:40:09
German foreign ministry it's the American
diplomats talking to the Russian foreign
00:40:14
ministry who talks to the Chinese
now the words it's not just
00:40:17
a question of addressing
00:40:18
a problem it's lateral communication around
the world is carried out by diplomats
00:40:22
and if you don't have the people who are
trained to do that you can miss signals
00:40:25
you can fail to inform people and it's
very hard to convince other people to work
00:40:30
with you if you haven't worked very hard
at developing trust with foreigners and
00:40:34
that they understand what you say it's
00:40:36
a question that makes people like me scratch
my head about how does one propose in
00:40:40
this administration to work with other
countries if the people who speak the
00:40:44
foreign languages know the people in
foreign countries are not there to pass on
00:40:48
those messages besides the fact that
we've got about 30 percent unfilled
00:40:53
ambassadorships right now there's also
that push to eliminate some $2300.00 jobs
00:40:58
from the State Department budget how
concerned are you about what is happening at
00:41:03
the State Department what would appear to
be it's being gutted to some degree let's
00:41:07
just try to look specifically at what
happens when you don't have people in these
00:41:11
jobs there are many people at the State
Department who are open minded enough to
00:41:15
say there are structural changes that
could be made at state that might get an
00:41:18
official For example many career people
have said many career diplomats have said
00:41:23
we have too many special envoys
and maybe a special envoy for x.
00:41:26
Or the special envoy for that doesn't need
to be named it can be that that function
00:41:30
can people get back into the bureaus fine
I don't think everyone is against any
00:41:35
change but the 2 things about what you've
said that I think are very important to
00:41:39
our ambassador real posts that are left
vacant give an impression to the country
00:41:44
where they are vacant that that country
doesn't matter now you can one can argue
00:41:47
that in the modern age you know ambassadors
are old fashioned and we have modern
00:41:51
communications but most countries take the.
American ambassador very seriously and
00:41:56
if you don't have an ambassador that is
noticed by that country and then there's
00:42:01
not only the symbolism but then secondly
there's the actual role what if there is
00:42:05
a problem in an important NATO ally like
Germany and you don't have an ambassador
00:42:10
there or. China what is the
mechanism by which you make sure
00:42:17
that the leader of that country that you
know that we're really getting our message
00:42:22
across and it's not going to be
misunderstood but if there's not a center
00:42:25
a place where the policies are discussed
and debated and communicated what happens
00:42:31
is it becomes
00:42:32
a very iffy use world within which
many people don't know which ideas are
00:42:38
authoritative or not and when you don't
know what ideas are authoritative it simply
00:42:42
becomes confusing and I
wouldn't want to think that in
00:42:46
a situation where you have tensions like
we do at the present with Russia or
00:42:50
tensions over North Korea I
would really not want just
00:42:54
a barrage of different opinions out there
I would like to think that people can say
00:42:58
well at least the authoritative version
from the State Department is this and it's
00:43:02
hard in some cases to find out when you
haven't filled staffed and trusted
00:43:08

Central progress has really been made over
the last 30 years in responding to the
00:44:30
epidemic and we can really now for the 1st
time probably ever talk about the end of
00:44:36
a we have all the tools that are necessary
or nearly all the tools that are
00:44:42
necessarily We certainly have the knowledge
and the understanding about what it
00:44:46
takes to end the epidemic and what we
really need now is the political will in
00:44:53
order to do so it's going to take resources
clearly knowing that we could reach and
00:44:59
it really is
00:45:00
a matter of result to figure out if we can
get there what will it take to get there
00:45:05
really one of the most important things
related to ending the epidemic would be
00:45:10
reducing new infections we have bio medical
interventions that clear. Really reduce
00:45:16
new infections including treatment
of HIV And what we what we now know
00:45:22
through various studies is that if individuals
can achieve and undetectible viral
00:45:28
load to where they have virus in their
body but that virus is not active we can
00:45:34
clearly demonstrate that there are no
infections that result the 2nd piece is for
00:45:40
people who are HIV negative there is
an intervention called pre-exposure
00:45:44
prophylactic or prep and that is all about
preventing new infections by also taking
00:45:51
anti-retroviral medication
just like somebody who's
00:45:54
a positive takes medication individual who
is negative if they take the currently
00:45:59
approved medication for prep it is
really a very low chance of becoming
00:46:06
a HIV positive as a result of the
exposure I understand that there's
00:46:11
a big meeting coming up with the aids
advocates and the Presidential Advisory
00:46:16
Council I know that this past number of
those advisers resigned from the council in
00:46:22
protest they accuse the president of not
caring enough about each of the issues and
00:46:26
this would be I am assuming the 1st time
the council has met since the resignations
00:46:31
What do you expect from this they are
meeting August 30th here in Washington d.c.
00:46:36
I expect that the council will continue
the results forward recommending to the
00:46:40
president that we continue the focus on
working towards the end of the epidemic I
00:46:47
think that one of the clear important
messages is that we still have
00:46:52
a lot of work to do we still have some
huge disparities in terms of who accesses
00:46:56
health care and who can achieve undetectible
viral load status for instance and so
00:47:02
I think the result is that we
really need ongoing support we need
00:47:08
resources for that we need
00:47:10
a concerted effort towards achieving the
national goals that have been. But in
00:47:15
place and I think you know I think that
the council will stay resolved in that in
00:47:21
that effort moving forward
there's also a meeting
00:47:25
a real important meeting coming up in
September many many advocates coming together
00:47:30
it's the u.s. Conference on Aids
and I expect that that will be
00:47:33
a similar push what we really need to
see from an appropriation standpoint is
00:47:38
Congress continuing to maintain their
appropriations for funding the Ryan White
00:47:44
program necessary services and we are
pleased that Congress continues to move in
00:47:51
that direction with the new fiscal year
about to begin another 2 months Congress
00:47:56
will need to pass some spending bills
continue to and can to continue funding
00:48:01
important programs that would benefit people
living with HIV Aids How concerned are
00:48:06
you that there could be spending cuts that
could be harmful to the progress that's
00:48:10
been made very concerned in the president's
budget propose some very significant
00:48:14
cuts to programs that provide important
benefits to people living with
00:48:18
a clue to. The National
Institutes of Health c.d.c.
00:48:23
The secretary's minority aids initiative
in the Ryan White program and if we were
00:48:28
to see those funding cuts implemented
it really would devastate entire
00:48:34
infrastructure that's been built and put
in place over much of the last 30 to 35
00:48:39
years and so as Congress continues their
work as you as you mentioned towards the
00:48:45
funding for fiscal year 18 we really need
to see funding levels maintained and
00:48:51
actually some of the cuts were actually
it put in place in 2017 restored part
00:48:57
c. And why did HIV Aids program
had a cut and the C.D.C.'s s.t.d.
00:49:02
Prevention program which is very closely
related to HIV programming those cuts need
00:49:08
to be restored we have been hearing much
lately about the Affordable Care Act It
00:49:11
seems for now it's staying in place although
there has been. Talk of the president
00:49:16
cutting funding for it which
would probably create quite
00:49:20
a few problems how would this impact
benefits people receive for hiv aids the
00:49:25
health care coverage is critical for people
living with HIV It really is time for
00:49:30
us to come to some kind of
00:49:32
a bipartisan solution that protect and
improve the health care of all Americans
00:49:37
including those living with h i
00:49:39
b we can't keep playing politics with

State Department is being dismantled to

some degree it does seem that there are
00:37:36
a number of slots under Secretary Ridge
which have not been filled there are
00:37:40
a number of spots which of the assistant
secretaries the people who are the
00:37:43
so-called regional bureaus these are the
people who really make policy views of the
00:37:47
close advisors to the secretary and normal
State Department and what's puzzling to
00:37:52
people like me who look you know whether.
Or not there's been Democrats or
00:37:55
Republicans these people are the people
trained in diplomacy especially the career
00:38:00
people but also the pull of political
people who come in and they're trained in
00:38:03
a mechanism which is part of the worldwide
mechanism which allows countries to deal
00:38:08
with one another on things from climate
change to nonproliferation to bilateral
00:38:12
issues to political alliances and so the
feeling that one gets and I'm as puzzled
00:38:18
by this as anyone else is that the leadership
of the government that it's the both
00:38:22
the White House and perhaps even Mr Tillerson
seem to think that changing the way
00:38:28
not changing the policy but changing the
way policy is made is something that's
00:38:31
very necessary and that they would
appoint people until those changes the
00:38:35
structural changes are made the difficulty
here is that there seems to be an
00:38:40
assumption that what diplomats do what
they're trained to do and how they do it is
00:38:45
somehow not as important as having different
ideas and again the analogy that I
00:38:51
struggle to find is you know would you
want someone who is who doesn't have
00:38:55
a driver's license to turn or I'm trying
to say that the people who might come in
00:38:58
would be inept It's just that
their various skills that
00:39:02
a diplomat has Do you think sort of
00:39:04
a control thing that this administration
just doesn't not want anyone from any past
00:39:08
administration this whole
loyalty thing we've heard
00:39:11
a lot from this White House do you think
that that could have something to do with
00:39:15
it and does loyalty even play
00:39:17
a role in this type of situation this to
me is beyond loyalty every group that has
00:39:22
come in famously Republicans turning over
to Democrats rather Democrats turning
00:39:26
over to Republicans initially they look
at the State Department and say These
00:39:30
people worked for my predecessor I just
ran against my predecessor and then they
00:39:34
quickly realize that when
you're trained to be
00:39:36
a diplomat you're serving the country and
you've oily and very carefully and very
00:39:40
honestly serve the person who is your boss
that's your constitutional job and very
00:39:45
quickly everyone I've worked with and I
worked in the Clinton White House and then
00:39:49
went on to work in the Bush White House
they quickly realize that they have very
00:39:53
valuable piece. We're going to give them
honest straightforward vice there's
00:39:57
another element to this which is not just
00:39:59
a domestic element the way in which our
country talks to 3rd countries let's say
00:40:03
there's an issue about missiles in North
Korea or the war in Syria who talks to the
00:40:09
German foreign ministry it's the American
diplomats talking to the Russian foreign
00:40:14
ministry who talks to the Chinese
now the words it's not just
00:40:17
a question of addressing
00:40:18
a problem it's lateral communication around
the world is carried out by diplomats
00:40:22
and if you don't have the people who are
trained to do that you can miss signals
00:40:25
you can fail to inform people and it's
very hard to convince other people to work
00:40:30
with you if you haven't worked very hard
at developing trust with foreigners and
00:40:34
that they understand what you say it's
00:40:36
a question that makes people like me scratch
my head about how does one propose in
00:40:40
this administration to work with other
countries if the people who speak the
00:40:44
foreign languages know the people in
foreign countries are not there to pass on
00:40:48
those messages besides the fact that
we've got about 30 percent unfilled
00:40:53
ambassadorships right now there's also
that push to eliminate some $2300.00 jobs
00:40:58
from the State Department budget how
concerned are you about what is happening at
00:41:03
the State Department what would appear to
be it's being gutted to some degree let's
00:41:07
just try to look specifically at what
happens when you don't have people in these
00:41:11
jobs there are many people at the State
Department who are open minded enough to
00:41:15
say there are structural changes that
could be made at state that might get an
00:41:18
official For example many career people
have said many career diplomats have said
00:41:23
we have too many special envoys
and maybe a special envoy for x.
00:41:26
Or the special envoy for that doesn't need
to be named it can be that that function
00:41:30
can people get back into the bureaus fine
I don't think everyone is against any
00:41:35
change but the 2 things about what you've
said that I think are very important to
00:41:39
our ambassador real posts that are left
vacant give an impression to the country
00:41:44
where they are vacant that that country
doesn't matter now you can one can argue
00:41:47
that in the modern age you know ambassadors
are old fashioned and we have modern
00:41:51
communications but most countries take the.
American ambassador very seriously and
00:41:56
if you don't have an ambassador that is
noticed by that country and then there's
00:42:01
not only the symbolism but then secondly
there's the actual role what if there is
00:42:05
a problem in an important NATO ally like
Germany and you don't have an ambassador
00:42:10
there or. China what is the
mechanism by which you make sure
00:42:17
that the leader of that country that you
know that we're really getting our message
00:42:22
across and it's not going to be
misunderstood but if there's not a center
00:42:25
a place where the policies are discussed
and debated and communicated what happens
00:42:31
is it becomes
00:42:32
a very iffy use world within which
many people don't know which ideas are
00:42:38
authoritative or not and when you don't
know what ideas are authoritative it simply
00:42:42
becomes confusing and I
wouldn't want to think that in
00:42:46
a situation where you have tensions like
we do at the present with Russia or
00:42:50
tensions over North Korea I
would really not want just
00:42:54
a barrage of different opinions out there
I would like to think that people can say
00:42:58
well at least the authoritative version
from the State Department is this and it's
00:43:02
hard in some cases to find out when you
haven't filled staffed and trusted

of Aids today in the u.s.

Central progress has really been made over
the last 30 years in responding to the
00:44:30
epidemic and we can really now for the 1st
time probably ever talk about the end of
00:44:36
a we have all the tools that are necessary
or nearly all the tools that are
00:44:42
necessarily We certainly have the knowledge
and the understanding about what it
00:44:46
takes to end the epidemic and what we
really need now is the political will in
00:44:53
order to do so it's going to take resources
clearly knowing that we could reach and
00:44:59
it really is
00:45:00
a matter of result to figure out if we can
get there what will it take to get there
00:45:05
really one of the most important things
related to ending the epidemic would be
00:45:10
reducing new infections we have bio medical
interventions that clear. Really reduce
00:45:16
new infections including treatment
of HIV And what we what we now know
00:45:22
through various studies is that if individuals
can achieve and undetectible viral
00:45:28
load to where they have virus in their
body but that virus is not active we can
00:45:34
clearly demonstrate that there are no
infections that result the 2nd piece is for
00:45:40
people who are HIV negative there is
an intervention called pre-exposure
00:45:44
prophylactic or prep and that is all about
preventing new infections by also taking
00:45:51
anti-retroviral medication
just like somebody who's
00:45:54
a positive takes medication individual who
is negative if they take the currently
00:45:59
approved medication for prep it is
really a very low chance of becoming
00:46:06
a HIV positive as a result of the
exposure I understand that there's
00:46:11
a big meeting coming up with the aids
advocates and the Presidential Advisory
00:46:16
Council I know that this past number of
those advisers resigned from the council in
00:46:22
protest they accuse the president of not
caring enough about each of the issues and
00:46:26
this would be I am assuming the 1st time
the council has met since the resignations
00:46:31
What do you expect from this they are
meeting August 30th here in Washington d.c.
00:46:36
I expect that the council will continue
the results forward recommending to the
00:46:40
president that we continue the focus on
working towards the end of the epidemic I
00:46:47
think that one of the clear important
messages is that we still have
00:46:52
a lot of work to do we still have some
huge disparities in terms of who accesses
00:46:56
health care and who can achieve undetectible
viral load status for instance and so
00:47:02
I think the result is that we
really need ongoing support we need
00:47:08
resources for that we need
00:47:10
a concerted effort towards achieving the
national goals that have been. But in
00:47:15
place and I think you know I think that
the council will stay resolved in that in
00:47:21
that effort moving forward
there's also a meeting
00:47:25
a real important meeting coming up in
September many many advocates coming together
00:47:30
it's the u.s. Conference on Aids
and I expect that that will be
00:47:33
a similar push what we really need to
see from an appropriation standpoint is
00:47:38
Congress continuing to maintain their
appropriations for funding the Ryan White
00:47:44
program necessary services and we are
pleased that Congress continues to move in
00:47:51
that direction with the new fiscal year
about to begin another 2 months Congress
00:47:56
will need to pass some spending bills
continue to and can to continue funding
00:48:01
important programs that would benefit people
living with HIV Aids How concerned are
00:48:06
you that there could be spending cuts that
could be harmful to the progress that's
00:48:10
been made very concerned in the president's
budget propose some very significant
00:48:14
cuts to programs that provide important
benefits to people living with
00:48:18
a clue to. The National
Institutes of Health c.d.c.
00:48:23
The secretary's minority aids initiative
in the Ryan White program and if we were
00:48:28
to see those funding cuts implemented
it really would devastate entire
00:48:34
infrastructure that's been built and put
in place over much of the last 30 to 35
00:48:39
years and so as Congress continues their
work as you as you mentioned towards the
00:48:45
funding for fiscal year 18 we really need
to see funding levels maintained and
00:48:51
actually some of the cuts were actually
it put in place in 2017 restored part
00:48:57
c. And why did HIV Aids program
had a cut and the C.D.C.'s s.t.d.
00:49:02
Prevention program which is very closely
related to HIV programming those cuts need
00:49:08
to be restored we have been hearing much
lately about the Affordable Care Act It
00:49:11
seems for now it's staying in place although
there has been. Talk of the president
00:49:16
cutting funding for it which
would probably create quite
00:49:20
a few problems how would this impact
benefits people receive for hiv aids the
00:49:25
health care coverage is critical for people
living with HIV It really is time for
00:49:30
us to come to some kind of
00:49:32
a bipartisan solution that protect and
improve the health care of all Americans
00:49:37
including those living with h i
00:49:39
b we can't keep playing politics with
people's lives all right that's Marie Panner
